For a Telehealth appointment, we ask you to download, print, and fill out the forms below. Specifically, Intake Form, Client-Psychotherapist Agreement, Acknowledgement of HiPAA, Release of Information, Telehealth Consent, and Payment Guarantee. You can email, fax, or mail the forms to your therapist.
